Step 1. Complete the form below to begin the layer swap process. (If you are not returning
              any layers, you can go straight to the new layer order form and skip this page.)


Step 2. Upon submitting this form, you will be taken to a screen to order your new layers.

Step 3. Once you receive your new layers, schedule a pickup for your original layers
           using a prepaid shipping label that will be emailed to you.

Step 4. Once we receive your returned layer(s) back, we will refund you for the new layers
           less any applicable shipping fees. We reserve the right to reject any returned layers
           that are found to be damaged or unsanitary. Please note that all layers must be
           returned within 90 days of your original invoice date.
